2, 2000 - Re-admitted to hospital to have surgery to remove 14"
metal
ribbon retractor. Discovery and further surgery for flesh-eating bacteria
(necrotizing fasciitis).

13, 2000 - Discharged from hospital. Home-health nurse care begins
two-a-day
treatment of bleach and gauze packing inside wound to treat abdominal
infection.

5, 2001 - Visits to wound care center for continuing treatment
of open abdominal wound.

11, 2001 - Plastic reconstruction of abdominal wound.

2, 2002 - Terminated from employment due to permanent
disability.

13, 2002 - Emg and sensory nerve testing by board certified
physician in neurology & electromyography revealed peripheral
neuropathy of both lower extremities due to sensory nerve damage.
